54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted from 9:45 a.m.-10:45 a.m. with the provider. Six children, ages 14 months, 14 months, 2 years, 3 years, 4 years, and 4 years old, were under the care and supervision of two caregivers for a total of 16 points. Children were observed playing with Play-Doh and engaged with various toys and activities such as the pretend kitchen, Duplo blocks, and the playhouse. The last emergency escape drill was 11/3/16. The provider and assistant are both currently certified in first aid and CPR. The battery operated radio and flashlights were present and accessible. One household member's and two children's records were reviewed and complete. One staff's record was reviewed. Standard #: 22VAC40-111-120-B Description: Based on review of two staff records, a staff member did not have two reference checks completed before employment. Evidence: Staff A's (hired 11/9/16) record did not include documentation of two or more reference checks. Plan of Correction: The provider stated that she will call and check two references for Staff A today.
