Technical Assistance:
Online Pre-Service for Subsidy is required to maintain the vendor agreement for the Subsidy program - the ten hours of training can count toward required annual training hours.

Comments:
An unannounced monitoring inspection was conducted with provider and daughter. There were three day care children and two residential children present, ranging in age from 11 months to 3 years old, totalling 14 points. The children were waking from rest and playing with age appropriate toys. Five children's files and three household files were reviewed - please see Supplemental Summary for missing items. Provider's first aid and CPR certification is valid until 1/9/2018. Provider does not administer medication to the day care children - Medication Administration Training is required to administer medicine. Standard #: 22VAC40-111-100-A
Description: Based on review of five children's files, the provider failed to have documentation of a physical on file for each child within 30 days of first attendance. Evidence: Child A (date of enrollment 9/12/2016) did not have documentation of a physical.

Plan of Correction: Provider will request this information from the parent again.
