Online Pre-Service for Subsidy is required to maintain the vendor agreement for the Subsidy program - the ten hours of training can count toward required annual training hours.
An unannounced monitoring inspection was conducted with provider and daughter. There were three day care children and two residential children present, ranging in age from 11 months to 3 years old, totalling 14 points. The children were waking from rest and playing with age appropriate toys. Five children's files and three household files were reviewed - please see Supplemental Summary for missing items. Provider's first aid and CPR certification is valid until 1/9/2018. Provider does not administer medication to the day care children - Medication Administration Training is required to administer medicine. Standard #: 22VAC40-111-100-A Description: Based on review of five children's files, the provider failed to have documentation of a physical on file for each child within 30 days of first attendance. Evidence: Child A (date of enrollment 9/12/2016) did not have documentation of a physical. Plan of Correction: Provider will request this information from the parent again.
