Technical Assistance:
We discussed the requirements for caregiver records, and diapering procedures. The risk assessments for violations was provided. We talked about the options for obtaining a minimum of sixteen hours of training each year.

Comments:
Thank you for your assistance during today' s unannounced monitoring inspection conducted from 11:12 AM to 2:55 PM. Today, there were twelve children (thirty-eight to forty points) with three caregivers. I viewed staffing, supervision, interactions with children, capacity, posted information, equipment, diapering procedures, emergency supplies and drill records, four records for children, lunch/menu, four caregiver/resident records, caregiver training, daily routines, activities, indoor and outdoor areas. The children's records were found to be organized and complete. The caregivers were observed responding to individual needs for comfort, care and assistance. A large selection of age appropriate toys and equipment were available in an organized, clean environment. Let me know if you have any questions. 540/430-9259

Standard #: 22VAC40-111-180-A
Description: Based on a review of four records for caregivers/residents, and an interview with the provider, the updated TB statements for caregiver 1 and 4 were not obtained by two years from the date of the last TB screening and statement. Evidence: The most recent TB statement for caregiver 1 was from 12/05/2014. The TB statement in the file for caregiver 4 was from October 11, 2014.

Plan of Correction: The provider will make arrangements for the needed updated TB screenings and statements. The provider will make sure the updated TB screenings and statements for other caregivers/residents before two years from the date of the last statements.

Standard #: 22VAC40-111-690-E
Description: Based on observation of the diaper changing procedures, and interviews with caregivers, a wet diaper and used wipes were disposed of in the lined container without the use of the foot-pedal to open the lid. Evidence: During a diaper change after lunch, the caregiver was seen lifting the lid of the container with a hand while disposing of the diaper and then the used wipes. The only time the the foot-pedal was used to lift the lid was when the disposable towels that were used to clean and then sanitize the changing surface were dropped into the opened container.

Plan of Correction: The caregiver will make sure the foot-pedal of the disposal container is within reach prior to beginning the diaper change of individual children so that the caregiver's hands and the used diapers and wipes have no contact with the surface of the disposal container. The provider will make sure all caregivers understand the disposal procedures to prevent the spread of germs.
