Today an unannounced inspection was conducted with Provider. This visit took place in the morning between 11am and 12:45pm. There were 12 children present with the Provider and 2 assistants for a total of 29 points within the supervision guidelines. The children were observed having free play, diapering, hand washing and transition to lunch. No medications are provided. All staff records were reviewed and 5 children's records were reviewed. Positive interactions were observed during this inspection. A complete inspection of the areas designated for programning, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-830-B Description: Based on review of emergency response drills, shelter in place procedures were not practiced. Evidence: Provider did not complete the minimum two shelter in place drills as required. Plan of Correction: Provider will begin a shelter in place drill this month.
Standard #: 22VAC40-111-100-A Description: Based on review of children's records one physical not found within 30 days of attendance. Evidence: Child #1 began care on 07/18/2016, no physical maintained in record. Plan of Correction: Provider will request physical for this child today from the parent. Will obtain by Friday and maintain in the record.
Standard #: 22VAC40-111-180-A Description: Based on review of all staff records, two TB report was not obtained every two years as required. Evidence: Staff #1's most recent TB report is dated 5/2014. Staff #2's most recent TB report is dated 9/2014. Plan of Correction: Both staff will obtain documentation of TB report. Both TB tests were completed but will obtain documentation and maintain in record.
Standard #: 22VAC40-111-210-A Description: Based on review of all staff records, no documentation found for the required 16 hours of annual training found. Evidence: All staff records were reviewed, no documentation found for any training provided in 2016. Plan of Correction: Staff training was obtained. Provider will request certificates for verification.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review of staff records, a central registry finding was not obtained within three years since the last search. Evidence: Staff #1's last central registry check was dated 11/2013. A current central registry check is not maintained. Plan of Correction: Provider will request another search.
