Technical Assistance:
1. Reviewed written injury report completion requirements. 2. Recommend outlets in bathroom have protective covers, as the tamper resistant internal cover is not visible and not determined if working properly.

Conducted an unannounced monitoring inspection 10:00-11:45am. Observed 10 children = 23 points with the Provider + Assistant. Ratios and supervision were in compliance. Children were eating snack, having story time and indoor free-play. The Provider stated that children had just come inside from active outdoor play in the fenced side yard area. Riding toys and climbing equipment have recently been added, and resilient surfacing was present. Caregivers were talking and playing with the children on the floor, and promptly tending to their needs. The daycare areas of the home were found to be clean and well supplied with age appropriate toys and equipment for the children. Diaper changing procedures were observed to be in compliance. No medications reported to be on-site. Required documents were posted. Record keeping violations found. Questions about this inspection may be directed to:

Description: Based on records reviewed, it was determined that 1 of 7 children's records did not include a signed acknowledgement from parent that emergency information was updated annually. Evidence: Child #19 - The parent signature was dated 1/23/16.

Plan of Correction: Will contact parent to complete updated emergency plan annually

Standard #: 22VAC40-111-60-B
Description: Based on records reviewed, it was determined that 1 of 7 children's records did not include the name, address, and telephone number of 1 of 2 emergency contacts. Evidence: Child #19-Space on form was left blank.

Plan of Correction: Will contact parent to complete emergency contact info

Standard #: 22VAC40-111-70-A
Description: Based on records reviewed, it was determined that 1 of 7 children's records did not include a completed Information for Parents form. Evidence: Child #1 - Page 2 of the document was left blank, and did not include termination policy and substitute caregiver information.

Standard #: 22VAC40-111-340-C
Description: Based on observations made, it was determined that there were no paper towels in the daycare bathroom. Evidence: A cloth towel was being used for drying hands.

Plan of Correction: Will place paper towels to replace towels

Standard #: 22VAC40-191-40-D-1-c
Description: Based on records reviewed, it was determined that the Provider did not have a Criminal Record Check (CRC) every 3 years. Evidence: The previous CRC was completed on 1/31/14, and a new CRC has not been obtained.

Plan of Correction: I will resend correct form to VA Police since previous form sent was not correct.
