22VAC40-111-(7)-520-B Cribs shall not be used as a play space for an infant.
An unannounced monitoring inspection was conducted on March 15, 2017 from approximately 10:00 AM to 12:00 PM. Provider and assistant were present with eight children. Children were observed playing freely, participating in circle time and eating lunch. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, nutrition, special feedings, emergency evacuation and shelter-in-place drill documentation, children's records, provider's record, household member's record, emergency supplies and required postings. Violations were reviewed with and verified by the provider during the exit interview conducted on this date.
Standard #: 22VAC40-111-830-B Description: Based on review, provider did not ensure shelter-in-place procedures are practiced a minimum of twice per year. Evidence: On March 15, 2017, there was documentation of one shelter-in-place drill in 2016 that was conducted in January 2016. Plan of Correction: Two shelter-in-place drills will be done this year. One has already been conducted so far.
Standard #: 22VAC40-111-100-A Description: Based on review, provider did not ensure 1 of 3 children's records has a physical examination prior to child's attendance or within 30 days of beginning attendance. Evidence: On March 15, 2017, the record for child #1 (start date January 30, 2017) did not have a physical. Plan of Correction: Provider will ask parent again for the physical.
Standard #: 22VAC40-111-60-B Description: Based on review, provider did not ensure 3 of 3 children's records have documentation of required information. Evidence: On March 15, 2017, the following was found during record review: 1. The record for child #1 (start date January 30, 2017) did not document mother and father's employment address, mother's employment phone number and the second emergency contact address. 2. The record for child #2 (start date January 9, 2017) did not document a complete address for the second emergency contact. 3. The record for child #3 (start date March 2, 2017) did not document mother and father's employment address as well as the second emergency contact address. Plan of Correction: Provider will get information from the parents.
Standard #: 22VAC40-111-250-C Description: Based on observation, provider did not ensure toys hung from the sides of cribs with strings or cords are removed when the child begins to push up on hands and knees or is five months of age, whichever occurs first. Evidence: On March 15, 2017, child #1 was observed in a crib with a stuffed toy tied with strings on the side of the crib. The child was observed laying on her stomach and pushing up on her hands and knees as well as sitting up and playing with a toy. Plan of Correction: The toy will be removed.
