Technical Assistance:
Reviewed free training opportunities and e-mailed the link for the pre-service training flyer as well as the model form for field trips.

Comments:
An unannounced monitoring visit was conducted during the Conditional licensure period. Five children were attending the day care but only three were at the home during the inspection as two were at the local playground. One provider was present with the three smaller infants for twelve points which is within ratios for supervision. Children were observed in infant care and free play as well as one child sleeping in a crib on the second level of the home. Reviewed a child's record, emergency preparation, nutrition and administrative records. The provider has current first aid and CPR certification. Household pets are current for rabies and are kept in the basement during the day. Areas of non-compliance are identified in this report. Description: Based on provider statement and evacuation log, monthly evacuation drills have not been conducted for 3 months. Evidence: Drills have not been conducted for the months of November 2016, December 2016 and February 2017.

Description: Based on provider statement, an evacuation drill for January 2017 was conducted but not documented. Evidence: The evacuation log did not contain documentation of a drill conducted in January 2017 but the provider stated one had been conducted with the children.
