- Technical Assistance:
Discussions held with the provider today on the following topics: Soon to expire negative Tuberculosis screenings of household members. Retaining the manufacturer's original manuals and documents for fixtures used by children in the home to verify compliance with U.S. Consumer Product Safety Commission standards.
An unannounced renewal inspection was conducted today, April 6, 2017 from 11:15am until 1:20pm. The inspector observed 11 children in care totaling 37 points under the direct supervision of 3 caregivers. The children were observed eating a lunch of fish sticks, french fries, bread and milk. The children were also observed participating in a dancing activity, reading to self, and transitioning to nap. The inspector observed caregivers helping young children wash their hands. The inspection included a review of 3 staff, 1 household member and 2 children's records. The home, medications, the emergency plan and emergency drills were also inspected. The home was clean, organized and abundantly supplied with books and learning materials. Areas of non-compliance are noted in the violation notice. Standard #: 22VAC40-111-710 Description: Based on medication review, a child's emergency medication did not contain a prescription label with the child's name or prescriber's instructions pertaining to dosage, frequency, and manner of administration. Evidence: Two doses of pre measured auto- injectors of epinephrine (belonging to Child#1) were observed loose in a zip lock bag without a prescription label or manufacturers instructions. Plan of Correction: "In line with this day care's policy, the ziploc bag containing 2 EpiPen injections along with the health provider's name and authorization letter has been returned to the parent. The parent understands the day care policy of offering a nut free environment as well as calling 911 in an emergency."
Standard #: 22VAC40-111-920 Description: Based on observation and paperwork review, the posted menu did not list substituted foods. Evidence: The posted lunch menu stated that fish, tortilla, peas and carrots and milk were to be served. The children were observed eating fish, bread and french fries and milk. Plan of Correction: "Whole wheat bread was offered in place of tortilla and potato instead of peas and carrots alomg with fish and milk. Provider's understanding was that she was not excluding any food group."
Standard #: 22VAC40-111-120-B Description: Based on record review, 1 of 3 staff records checked were incomplete. Evidence: Staff #2's record contained one of the two required reference checks. Plan of Correction: "The said staff member has been with the day-care for nearly 5 years and her file has undergone multiple inspections without any notation of a missing document."
Standard #: 22VAC40-111-210-A Description: Based on record review, 0 of 3 caregivers completed the required 16 hours of annual training. 1. Staff #1 has completed 12 of the required 16 hours of training between 2016-2017. 2. Staff #2 has completed 11 of the required 16 hours of training between 2016-2017. 3. Staff #3 has completed 11 of the required 16 hours of training between 2016-2017. Plan of Correction: "All three staff will have completed their training hours before license renewal by enrolling in classes offered in April and early May."
Standard #: 22VAC40-111-220-B Description: Based on record review, not all caregivers required to have medication administrative training had repeated the training in the required three year intervals. Evidence: Adult #1's last documented medication administrative training was conducted on September 7, 2013. Plan of Correction: "This day-care follows a policy of not administering medication- prescription or nonprescription, hence MAT is not required."
