Comments:
An initial study was conducted from 11:00 a.m.-2:00 p.m. with the provider. There were no children in care during the inspection. Two children are currently enrolled. The provider holds current certification in first aid and CPR. The provider has two rooms designated for the daycare, and both rooms are well-stocked with equipment and materials for the care of children. Two household members' records were reviewed and complete. Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the provider did not maintain a complete first aid kit. Evidence: The first aid kit did not have triangular bandages.

Plan of Correction: The provider stated that she will order triangular bandages online tonight.

Standard #: 22VAC40-111-770
Description: Based on observation, the provider did not have a battery-operated weather band radio. Evidence: The provider had a solar crank weather band radio.

Plan of Correction: The provider stated that she will purchase a battery-operated weather band radio online tonight.

Standard #: 22VAC40-111-800-A
Description: Based on observation and provider interview, the provider did not have a written emergency preparedness and response plan. Evidence: There was no written emergency preparedness and response plan for the family day home.

Plan of Correction: The provider stated that she will complete the written emergency preparedness and response plan.

Standard #: 22VAC40-111-60-A
Description: Based on observation and provider interview, the provider did not have records for each enrolled child. Evidence: Child A and Child B did not have records.

Plan of Correction: The provider stated that she will notify the parents to complete the enrollment packets.
