Technical Assistance:
1. Discussed process for moving and relocating the daycare to a new address. 2. Reviewed how/when to count visiting children in daycare numbers.

Comments:
Conducted an unannounced monitoring inspection 3:05-4:30pm. Observed 4 children = 12 points with the Provider + Assistant. Ratios and supervision were in compliance. Children were taking naps, washing hands, eating snack, and playing outside during the inspection. While the older children were outside playing, the younger children had play time on the floor. Caregivers were talking with the children and tending to their needs. School-age children were observed arriving from school, washing hands, and eating snack at the conclusion of the inspection. Snack served: saltine crackers, cream cheese, broccoli, and milk. The daycare areas were sufficiently supplied with toys and equipment for the children. No medications reported to be on-site. Standard #: 22VAC40-111-240-A
Description: Based on observations made, it was determined that a furnishing of the daycare was not maintained in a safe and operable condition. Evidence: A riding toy (Batman car) was observed to have broken plastic edges with sharp points.

Plan of Correction: The toy was removed from the outdoor backyard play area today, and will be thrown away.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on records reviewed, it was determined that the Assistant caregiver and and Adult #1 do not have completed background checks before 3 years since last completed. Evidence: 1. Assistant - Sworn Disclosure Statement (SDS) was dated 4/9/14, Criminal Record Check (CRC) was dated 3/11/14, and Central Registry Check (CPS) was dated 3/10/14. 2. Adult #1 - SDS was dated 11/19/13, CRC was dated 11/8/13, and CPS was dated 12/16/13.

Plan of Correction: I will send new background checks tomorrow.
