Comments:
An unannounced monitoring inspection was conducted on 04/19/2017 from 10:10am - 12:45pm. At the time of entrance there were seven children in care with two caregivers. The ages of children counted for a total of 18 points. Children were observed playing games, working with puzzles, singing songs and reading books with the provider and assistant. Morning snack, lunch and nap was also reviewed. The inspector reviewed the entire home and backyard play area. Records were reviewed for both caregivers and the seven children in care. *No medication is administered at this time. The violations were reviewed with the provider at the conclusion of the inspection and are listed on the violation notice.

Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, the provider failed to ensure that bottles shall be labeled with the child's full name and the date, if more than one infant is in care. Evidence: 1. Three bottles of infant formula were not labeled with the child's name and date. 2. Two bottles of infant formula were not dated. 3. These bottles were for two different infants who were in care during the inspection.

Plan of Correction: These bottles will be dated and labeled today.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider failed to ensure that potentially poisonous substances, materials and supplies such as, cleaning agents, shall be inaccessible to children. Evidence: 1. Three bottles of cleaning agents were observed stored in an unlocked cabinet in the kitchen. a. The child proof latch on the cabinet was broken. 2. This is a bottom cabinet that is accessible to children. Children were observed in the kitchen eating morning snack and lunch.

Plan of Correction: These products will be locked up today.
