Technical Assistance:
During inspection inspector reminded provider to get all required documentation for newly hired assistant.

Comments:
An unannounced monitoring inspection was conducted today from approximately 11:30am-12:40pm. There were 6 children(14 points) directly supervised by 2 staff. The home, 2 staff records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, watching television and preparing for rest. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on review of documentation emergency evacuations were not practiced monthly. Evidence: During inspection there was no documentation of evacuation drills being conducted for January, February and March of this year.

Plan of Correction: I forgot to write them out I will do it for this year.

Standard #: 22VAC40-111-130
Description: Based on inspection and conversation with provider all required information for caregivers was not maintained. Evidence: Caregiver providing care and recently hired did not have a signed Sworn Disclosure Statement available.

Plan of Correction: She is still here on a trail bases, I will get all of her paperwork.

Standard #: 22VAC40-111-170-A
Description: Based on inspection and conversation with provider each caregiver did not have a current Tuberculosis (TB) screening prior to contact with children. Evidence: During inspection the assistant providing care did not have a current TB screening on record.

Plan of Correction: She just started she will go to the clinic on Monday.

Standard #: 22VAC40-111-280
Description: Based on inspection cleaning agents were not inaccessible to children. Evidence: During inspection the laundry room in area where care is provided, containing cleaning supplies was unlocked.

Plan of Correction: Corrected after inspector mentioned it. Provider was reminded to keep that door lock at all times.
