Today an unannounced renewal visit was conducted with the Provider. This visit took place in the afternoon. There were 8 children present with the Provider and 1 assistant for a total of 26 points within the supervision guidelines. The children were observed having lunch, story-time, free play and nap time. Lunch consisted of pizza and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-920 Description: Based on observation of the menu, the menu did not reflect what was being served. Evidence: Upon arrival, the children were observed having lunch however what was being served was not posted on the menu. Plan of Correction: Provider stated that a parent provided lunch for the children today to celebrate a birthday and she did not have time to change the menu.
Standard #: 22VAC40-191-40-D-1-b Description: Based on a review of the provider records, one caregiver record did not include a criminal record check within 30 days of employment. Evidence: Caregiver #1's record did not include a criminal record check start date 3/1/17. Plan of Correction: Provider stated that she submitted it but it was returned along with a form stating that as of February 1st they only accept forms online. Provider is waiting the results and have submitted the form online.
