An unannounced monitoring inspection was conducted on April 25th 2017 between 11:00am and 1:30pm. There were 10 children in the direct care of 3 staff members. Children were observed washing hands, dancing, singing, and participating in circle time. Children were also read books and napping. The home is clean, organized, and offers plenty of space for children to utilize. A sampling of children's records, staff records, the physical space, supervision, the evacuation drills, the emergency plan, and cribs were observed and inspected. Standard #: 22VAC40-111-940-B Description: Based on observation, the provider did not have an operable thermometer to monitor refrigerator and freezer compartment temperatures. Evidence: The provider stated that the thermometer that was in the refrigerator recently broke. Plan of Correction: We will get a new one.
Standard #: 22VAC40-111-100-A Description: Based on record review, 2 of 4 records did not have documentation of a physical examination within 30 days of first attendance. Evidence: Child #1 and Child # 4 did not have documentation on file of a physical examination. Plan of Correction: I will ask the parents to bring a copy of it.
Standard #: 22VAC40-111-180-A Description: Based on observation, 1 of 4 staff records reviewed did not have documentation of a current report of Tuberculosis screening every two years since the date of the first report. Evidence: Current documentation could not be located for Staff #1. The last documented date of screening was July 2013. Plan of Correction: I will request a copy from the health clinic.
Standard #: 22VAC40-111-240-B Description: Based on observation, a piece of equipment was in use that had been recalled by the U.S. Consumer Product Safety Commission. Evidence: The item was recalled August 15th 2012. Plan of Correction: We will not use it.
