An unannounced monitoring inspection was conducted from 9:03 a.m. - 12:20 p.m. There were no children in care at the time of the inspection. There were two children in care earlier this morning who got on the bus. The total points were two. Two children's records were reviewed. The record for the provider was reviewed. Standard #: 22VAC40-111-60-B Description: Based on record review the facility failed to have written parent acknowledgment of the annual review of emergency contact information. Evidence: 1. On 05/03/2017 the licensing inspector reviewed the record for child #1 and child #2. There was no signed parent acknowledgment of the annual review of emergency contact information. 2. On 05/03/2017 the provider acknowledged she had not gotten the parents to sign off that the annual review of emergency contact information had been conducted. Plan of Correction: Both parents will be picking their child up this afternoon and I will have them sign off for the record.
Standard #: 22VAC40-111-140-B-3 Description: Based on record review and interview the provider did not have documentation of current CPR. Evidence: 1. On 05/03/2017 the licensing inspector requested to see documentation of the providers CPR training. The provider advised she had not been able to get her CPR training updated. The expiration of the last CPR training was February, 2017. Plan of Correction: I will schedule my training for next month with the American Heart Association.
Standard #: 22VAC40-111-140-B-4 Description: Based on record review and interview the provider did not have current first aid training. Evidence: 1. On 05/03/2017 the licensing inspector requested documentation of current first aid training. The provider advised she had not updated her first aid training. Plan of Correction: I will get my first aid training as soon as possible with the American Heart Association.
