Comments:
An unannounced SHSI monitoring inspection was completed on 05/03/2017. There were 6 children, ages 4 mo.-4 years, totaling 16 points in care today. There was 1 provider and household member present during the inspection. The inspector reviewed 2 children's records and 1 provider record. The children were observed during morning snack, free choice time and lunch. An exit interview to review areas of non-compliance was conducted at the conclusion of the inspection on 05/03/2017.

Violations:
Standard #: VENDHOM-000-012-A
Description: Based on the Licensing Inspector's review of documents and interview with the provider, the provider failed to complete the required training as stated in the standards. Evidence: The provider has not completed the required Virginia Preservice Training.

Plan of Correction: Will take as soon as possible.

Standard #: VENDHOM-000-026-A-1
Description: Based on the Licensing Inspector's observations, the provider failed to ensure that children washed their hands after eating meals or snacks as required by the standards. Evidence: The children did not wash their hands upon completion of their morning snack.

Plan of Correction: Will take care of today.

Standard #: VENDHOM-000-029-B-3
Description: Based on the Licensing Inspectors interview with the provider, the provider failed to ensure that there was a procedure to reunite children with a parent or authorized person designated by the parent to pick up the child in the event of an emergency as required by the standards. Evidence: The Emergency Preparedness Plan does not include a evacuation/relocation reunification plan.

Plan of Correction: Will take care of as soon as possible.

Standard #: VENDHOM-000-029-B-4
Description: Based on the Licensing Inspectors interview with the provider, the provider failed to ensure that there was a procedure to reunite children with a parent or authorized person designated by the parent to pick up the child in the event of an emergency as required by the standards. Evidence: The Shelter-in-place Plan does not include a shelter-in-place reunification plan.

Plan of Correction: Will take care of today.

Standard #: VENDHOM-000-029-B-5
Description: Based on the Licensing Inspectors interview with the provider, the provider failed to ensure that there were lockdown procedures. Evidence: The provider does not have lockdown procedures in place.

Plan of Correction: Will fix it today.
