An unannounced renewal inspection was conducted beginning at 11:10 and ending at 12:30. There were 4 daycare children and one child who lives in the home. Children were ages 5 months - 4 years. One additional school age child will be here after school. The provider's daughter was helping with the children. Observed children playing in the day care area , eating lunch of chicken nuggets, french fries and milk, bread. Reviewed all children and family files - see violation notice. Provider is not MAT trained and does not give medication. Emergency escape drills are up to date. If you have any questions. Standard #: 22VAC40-111-60-B Description: Based on a review of 5 children's files, it was determined that not all files were complete. Evidence: Five of 5 files had missing information as follows: Child #1 is missing Insurance policy number and physical - enrollment date 1/3/16; Child #2 is missing a physical - enrollment date 5/23/16; Child #3 is missing a second emergency contact and insurance policy number; Child #4 is missing a physical enrollment date 5/1/16; Child #5 is missing a shot record - enrollment date last week.
