Technical Assistance:
The following areas of the vendor agreement were reviewed: *Allergy plans for children *Attendance *Lockdown procedures and practice drills *Reunification plans to be included in the emergency plan. *Preservice Training for caregivers

The Licensing Inspector arrived at 10:10am and departed at 12:30pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were eight children, ages 6-months through five-years, in care with two caregivers. The morning preschool program and lunch was observed. The sample size was eight children and three caregiver/household member records.

Standard #: VENDHOM-000-005-B
Description: Based on record review, the vendor failed to maintain a written hard copy of daily attendance that documents the arrival of each child in care as it occurs. Evidence: 1. One child in care was not noted on the written attendance sheet for 5/12/2017. 2. The provider does have a written attendance sheet that meets the requirements of the vendor agreement however, one parent failed to sign their child in upon arrival.

Plan of Correction: Parent's will be reminded to sign in on the attendance sheet.

Standard #: VENDHOM-000-012-E
Description: Based on interview and record review, the vendor failed to ensure that all caregivers shall have completed Virginia Preservice Training for Child Care Staff within 90 days of employment. Evidence: 1. Caregiver #1 has not completed Virginia Preservice Training for Child Care Staff. 2. Caregiver #1 has a hire date of 10/12/2015.

Plan of Correction: Caregiver #1 has initiated this training and will work toward completion within 30 days.

Standard #: VENDHOM-000-029-B-5
Description: Based on record review, the vendor failed to ensure that the emergency plan for the family day home included lockdown procedures. Evidence: 1. The written emergency plan for the FDH did not include lockdown procedures. 2. The provider confirmed that lockdown procedures had not been written into the plan.

Plan of Correction: Lockdown procedures will be written and added to the current emergency plan within one week.
