Standard #: VENDHOM-000-012-C Description: Based on record reviews, the family day home failed to have caregivers provide documentation of completion of the preservice training. Evidence: 1. The inspector reviewed three caregiver's records. There were no certificates in the three caregiver records confirming that they had completed the ten hours of preservice training. Plan of Correction: All three caregivers completed the preservice training at the end of 2016, but the certificates have been misplaced. If the certificates cannot be found, all three caregivers will repeat the training. The training certificates will be sent to the licensing office.
