Discussion was held on the topics of current enrollment, current staffing, and completing other documentation for new staff person: orientation page, assistant/substitute provider record, and CPS (still under grace period) coming in the mail.
The following inspection took place between the hours of 1:30 pm through 4:15 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 9 children (24 points) with 3 caregivers. A total of 11 children?s files and 5 staff/household member?s files were reviewed. The children were engaged in the following: nap time, bathroom break, hand washing, diaper changing, snack time, art, music, song, dance, toy play, and using the fun room instead of going outside due to the weather, and other organized activities. Standard #: 22VAC40-111-60-B Description: Based on review, 3 of 11 children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (05/23/2017), 2 complete emergency contact information was not available for review for: Child #7, Child #10, and Child #11. 2. The child's insurance policy information was not available for: Child #10 (policy number), and Child #11 (both policy name and number). Plan of Correction: All missing information shall be obtain from each child's parent and placed on file.
Standard #: 22VAC40-111-180-A Description: Based on review, 2 of 5 staff/household member's files reviewed indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis (TB) Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence - 1. On the date of inspection (05/23/2017), the following members had TB documentation that was older than 2 years: Member #1 (03/31/2015), and Member #4 (03/31/2015). 2. The Provider confirmed this finding on the date of inspection. Plan of Correction: All applicable TB documentation shall be updated.
