**Reminder: background checks are due to be repeated this summer. The criminal history record checks are due 8/2017. Information regarding national fingerprint requirements will be send via e-mail over the next few weeks. If not finalized by August, you will need to conduct a criminal history record through the Va. State Police. The CPS checks are due 9/2017 - this procedure has not changed.

An unannounced monitoring inspection was conducted with provider. There were two resident children and four day care children, ranging in age from 1 to 8 years, present totalling 8 points. The children were having free play with age appropriate activities while lunch was prepared. Lunch served today: pork chops, broccoli, peaches, macaroni and cheese, and milk. After lunch, the younger children prepared for rest. Provider renewed first aid and CPR on 2/18/2017, which is valid for 2 years. Two children's files were reviewed - please see Supplemental Summary for missing items. There are no medications on site today. Description: Based on review of two children's files, the provider did not have a physical examination on file for each child within 30 days of child's first attendance. Evidence: Child A (date of enrollment 4/17/2017) does not have documentation of a physical on file.

Plan of Correction: Provider will request the physical from the parents again.
