54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring inspection was conducted beginning at 11:15 a.m. and ending at 12:30 p.m. with the provider and assistant. There were eight children in care ranging in age from nine months to five years old for a total of eighteen points. Children were observed having free play that included blocks and imagination play. After free play, children had coloring time. An infant was observed having play time in a play yard with various toys. Lunch will be pizza, baby carrots, oranges and milk. Three children's records, two staff records and one household member record were reviewed. The provider is Medication Administration Training (MAT) certified however no children in care had medications on site. Standard #: 22VAC40-111-940-A
Description: Based on observation, the temperature in the fridge in the chidlcare area was not maintained at or below 40 degrees Fahrenheit. The fridge temperature was 42 degrees Fahrenheit.

Plan of Correction: The fridge was turned down during the inspection. Usually I use the upstairs refrigerator for food storage.

Standard #: 22VAC40-111-960-E
Description: Based on observation, not all bottles were refrigerated as required. Evidence: One full bottle was observed on a table beside the fridge and a second bottle was on an ice pack in an insulated lunch bag.

Plan of Correction: The bottles were refrigerated during the inspection. In the future all bottles will be refrigerated as required.
