Technical Assistance:
New background check regulations to include the new sworn affirmation/disclosure statement were reviewed with the provider.

Comments:
An unannounced mandated monitoring inspection was conducted today. The inspector arrived at 11:05 AM and departed at 12:25 PM. Morning activities and lunch service were observed. A sample size of four children's records and caregiver training were reviewed. No children's injuries were reported. Medication administration was reviewed. An exit meeting was conducted with the provider prior to closure of the inspection. Please contact the inspector if there are any questions.

Violations:
Standard #: 22VAC40-111-280
Description: Based upon observation, the provider has not ensured that potentially poisonous substances are kept in areas inaccessible to the children. Evidence: There was a container of spray air freshener labeled "caution, keep out of reach of children" on a side table in the child care room.

Plan of Correction: The provider responded with the following: The air freshener was moved to an inaccessible/locked location during the inspection.
