Standard #: 22VAC40-111-770 Description: Based on observation of physical plant, emergency radio was not operable. Evidence: A battery operated radio was maintained in the provider's home but was not operable. Extra batteries were not maintained. Plan of Correction: I will replace the batteries in the radio and maintain extra batteries.
Standard #: 22VAC40-111-120-B Description: Based on staff record review, one record was not complete. Evidence: Staff #2 was hired in January 2017, only one documented reference was maintained. Plan of Correction: Provider will obtain a second reference for this staff.
Standard #: 22VAC40-111-200-C Description: Based on staff record review, orientation documentation was not maintained. Evidence: Staff #2 was hired in January 2017, no documentation found for orientation for this staff member. Plan of Correction: Provider verbally completed orientation training with this staff member but did not document the training. Provider will review the orientation with this staff member and maintain documentation in the record.
Standard #: 22VAC40-111-240-A Description: Based on observation of outdoor player, one area determined a hazard. Evidence: The wood planks located on the sides of the shed had splintered wood and was determined a hazard. These areas are accessible to children. Plan of Correction: I will make necessary repairs to these areas. The shed is on my property but belongs to the neighbor.
