Technical Assistance:
Dicussed the following standards: children's records, tuberculosis screening, and emergency procedures.

An unannounced renewal inspection was conducted today. Observed 5 children (10 points) directly supervised by 2 caregivers. A sample of 2 children's records and 2 caregiver records were reviewed today. No medications were reported to be on site. The children were observed during free play. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Inspection conducted from approximately 11:10am to 12:20pm. Standard #: 22VAC40-111-60-B
Description: Based upon documentation reviewed, children's records were incomplete. Evidence: 1) One of the two children's records reviewed did not contain the following: parent's work telephone number (Child A; Date of enrollment, address for the second emergency listed, parent's signed acknowledgment of the receipt of the medication administration policy (Child A; Date of enrollment (DOE) 01/2010). 2) One of the two children's records reviewed did not contain the name and policy number of the child's medical insurance (Child B; DOE 08/22/2017). 3) Two of the two children's records reviewed did not contain documentation verifying the child's age and identity (Child A and B).

Plan of Correction: I will ask mom during pick up to fill out the missing information.

Standard #: 22VAC40-111-90-A
Description: Based upon documentation reviewed and observations, the provider did not obtain documentation that a child has been immunized before the child attended the family day home. Evidence: 1) One of the two children's records reviewed did not contain immunizations record (Child B; DOE 08/22/2017). 2) Child B was present during today's inspection.

Plan of Correction: The parent is going to take the child to the doctor and I will obtain the document.

Standard #: 22VAC40-111-180-A
Description: Based upon documentation reviewed, the provider did not obtain for each caregiver a current Report of Tuberculosis (TB) Screening form within the allowable two years time frame. Evidence: One of the two caregiver records reviewed did not contain a current TB screening form; last TB form was dated 08/03/2015 (Caregiver #2).

Plan of Correction: I will get an update TB form by tomorrow.
