Description: Based on record review and interview, the provider did not complete annual review with the parent the required emergency contact information to ensure the information is correct. Evidence: 1. Child A (start date 09/14/15) and Child B (start date 09/14/2016) did not have documentation of an annual review of emergency contact information. 2. Provider stated he did not review emergency contact information with the parent of Child A or Child B.

Plan of Correction: I will ask the parents today.

Description: Based on record review, one child's records did not have complete information. Evidence: 1. Child A did not have the name and policy number of the child's medical insurance, proof of identity, name and address of previous child care.

Plan of Correction: I will fix it today and ask the parents.
