An unannounced renewal inspection was completed on 9/26/2017. There were seven children in care with the provider and an assistant supervising at a total of seventeen points. Five children's records, the provider's record and one household member's (assistant) record were reviewed. The inspector arrived at the provider's home at 9:50 A.M. and departed at 1:30 P.M. The children were observed during free play times, during morning snack, outside time, lunch and start of nap. There was discussion with the provider about the following: using household member as assistant and completing all required paperwork and training.

Standard #: 22VAC40-111-180-A
Description: Based on review of record, the licensee failed to ensure that the adult household member and the licensee the had a tuberculosis (TB) test within two years from the last test. Evidence: 1.The record of the household member was reviewed and the TB test was completed 7/23/2015. 2. The record of the licensee was reviewed and the TB test was completed on 4/29/2015.

Plan of Correction: The licensee stated that they will go and get TB screenings soon, and will monitor dates.

Standard #: 22VAC40-111-500-C
Description: Based on observation, the licensee failed to ensure that all outdoor play equipment had met the requirements by the standard. Evidence: 1. The composite play structure was approximately one foot from the climber. The required perimeter is 6 foot around each piece of equipment.

Plan of Correction: The climber will be moved and measured for the six foot required.

Standard #: 63.2-1720.1-B-3
Description: Based on review of records, the licensee failed to ensure that all required background checks were completed within the time frame. Evidence: 1. The licensee and household member (assistant) did not complete the new sworn statement that was required 9/1/2017.

Plan of Correction: The licensee will print and complete the new form today.
