Comments:
An unannounced monitoring inspection was conducted today beginning at 10:15am. 5 children (15 points) and 1 caregiver were present. Ratios were in compliance. The physical space, 5 children's records, provider and household member records, attendance logs and evacuation drills were reviewed. The provider is up-to-date with all trainings and required documents were posted. Children were observed collecting autumn leaves in the front yard and working on crafts. Two infants were observed napping. Cribs and diaper changing area were in compliance. The space was clean and contained an ample supply of age appropriate materials. Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not obtain all required information for the children's records. Evidence: One out of five records reviewed was incomplete. Child #1 was missing the medical insurance policy number and the Policy to Administer Medication was not signed.

Plan of Correction: I will ask the parent to complete the child's file today.
