An unannounced monitoring inspection was conducted on October 12, 2017. At the time of entrance there was 1 provider and 7 children with a total of 14 points. The sample size consisted of 5 children's records and 1 provider record. Children were observed during their afternoon nap and preparing for and eating their afternoon snack. The provider does not administer medications. The inspection began at approximately 2:00 p.m. and ended at 3:45 p.m. The results of the inspection were reviewed with the provider during the exit interview. Standard #: 22VAC40-111-180-A Description: Based on a record review and provider interviews, the provider did not obtain a current Report of Tuberculosis Screening form, every two years from the date of the first screening. Evidence: During the inspection on October 12, 2017, the most recent TB screening was dated for 6-24-2015. Plan of Correction: The provider will obtain the TB screening.
Standard #: 22VAC40-111-240-A Description: Based on observation, the provider did not ensure areas and furnishings of the family day home, inside and outside, were maintained in a clean, safe, and operable condition. Unsafe conditions include, but not be limited to, the presence of poisonous plants; tripping hazards; unstable heavy equipment, furniture, or other items that a child could pull down on himself; splintered, cracked, or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting, or breakage of any equipment; head entrapment hazards; and protruding nails, bolts, or other components that could entangle or could snag skin. Evidence: During the inspection on October 12, 2017, the licensing inspector observed the hooks connecting the swings to the metal chains on the swing set. Four of the six the hooks are rusting and the rust comes off easily when touched. Two of the six hooks connecting the swings are completely covered in duct tape. Plan of Correction: The provider will spray them and recover the hooks with duct tape.
Standard #: 22VAC40-111-290 Description: Based on observation, the provider did not ensure sharp kitchen utensils are inaccessible to children unless being used by the caregiver or with children under close supervision. Evidence: During the inspection on October 12, 2017, the licensing inspector observed a knife block with nine knives and a pair of scissors on the kitchen counter next to the stove and accessible to children. "Accessible" means capable of being entered, reached, or used. Plan of Correction: The provider removed the knifes as of 3:34 p.m today.
Standard #: 22VAC40-111-680-A Description: Based on observation, the provider did not ensure caregivers shall wash their hands with liquid soap and warm running water after diapering or assisting a child with toileting. Evidence: During the inspection on October 12, 2017, the licensing inspector observed the caregiver assist child #1 with toileting and did not wash their hands with soap and water after assisting the child. The caregiver was then observed serving the afternoon snack. Plan of Correction: The provider said Child #1 "was the only child that she did not wash their hands after helping him. The provider gave two plates of food to children and then took another child to the bathroom and washed her hands after helping that child". Caregiver said she washes her hands daily.
