An unannounced monitoring inspection was completed on October 17, 2017. The purpose of this visit was to measure the facility?s compliance with the licensing standards. There were 5 children in care, for a total of 16 points with two caregivers.
At the onset of the inspection, the children were eating a lunch of sweet peas, macaroni with sauce and cheese and either fruit cocktail or applesauce. Seconds of food was given to the children upon request. Later, the children's diapers were changed and they were put down to rest for nap. The facility administers medication and the provider is MAT certified. The facility does not provide transportation. The last evacuation drill was conducted 9/7/17. The most recent shelter-in-place was conducted 8/23/17. Two caregiver records and 5 children?s records were reviewed.

Standard #: 22VAC40-111-60-B
Description: Based on a review of a sample of children's records, the facility did not ensure that a child's record contained written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. EVIDENCE: The record of child#1 did not contain written authorization for emergency medical care.

Plan of Correction: I will have the parent sign the form tomorrow morning.

Standard #: 22VAC40-111-70-B
Description: Based on a review of a sample of children's records, the facility did not ensure that a child's record contained a parent's written acknowledgement of the receipt of required information. EVIDENCE: The record of child #1 did not contain the parent's written acknowledgement of the receipt of information regarding whether or not the facility has liability insurance.

Plan of Correction: I will have the parent sign the form tomorrow morning.
