A Subsidy Health and Safety inspection was conducted on October 20, 2017 to review supplemental health and safety requirements. There were 3 school-age children in care, the points totaled 3. The children arrived to the FDH close to 4:00 pm for evening care. The Provider prepared supper for the children. The children were observed in one of the child care areas during self-interest play. A sample of records was reviewed. The Provider reported medications are not administered. As reported, there are no children in care with a diagnosed food allergy. A licensing inspection was also conducted this date; the two combined inspections cover all subside health and safety requirements. Only subside requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. The inspection was conducted from approximately 2:30pm to 5:45pm. Findings of the inspection were reviewed with the Provider at exit. Thank you.

Description: Based on Provider report, the required VA Preservice Training has not been completed. Evidence: The Provider reported that she had started the on-line training but has not completed the training.

Plan of Correction: The Provider will continue to work on the training and will complete the10 hours.

Standard #: VENDHOM-000-029-B-5
Description: Based on Provider report, the Provider failed to ensure written lock down procedures were included in the FDH's emergency preparedness plan. Evidence: The LI requested to review the written lock down procedures, the procedures were not available for review.

Plan of Correction: The Provider will develop procedures for lock down.
