Comments:
An unannounced monitoring inspection was conducted today from approximately 9:50am-12:00pm. There were 2 children (8 points) directly supervised by 1 caregiver. A sample of 2 children?s records, the provider's record and 2 household member's records were reviewed. The children were observed being fed bottles, watching children's programming on television and during diapering procedures. Zero new staff have been hired since the last inspection. Standard #: 22VAC40-111-60-A
Description: Based on record review, children's files were incomplete. Evidence: 2 of 2 records reviewed were missing required information. 1) Child A was missing the work phone number of both guardians, 2 of 2 emergency contacts, and people authorized to pick child up. 2) Child B was missing the phone number of the father, and start date.

Plan of Correction: Parents will complete the files.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the tuberculosis screening was not renewed after the required 2 years. Evidence: 1) Household member A's tuberculosis test was last completed 10/20/2015 and expired 10/20/2017. 2) Household member B's tuberculosis test was last completed 02/17/2015 and expired 02/17/2017.

Plan of Correction: My husband did not return the paper to me.

Standard #: 22VAC40-111-330-A
Description: Based on observation, the landline phone present was not in compliance with the regulations. Evidence: The landline phone present was a cordless phone needing electricity to operate.

Plan of Correction: I will buy another one.

Standard #: 22VAC40-111-340-C
Description: Based on observation, the bathroom did not contain paper towels. Evidence: The bathroom on the level of the entrance did not contain paper towel.

Plan of Correction: Corrected during inspection.

Standard #: 63.2-1722-B
Description: Based on record review, background checks were not obtained. Evidence: Household member C did not have a CPS on file.

Plan of Correction: I will send it out.
