Licensing inspector spoke with provider about Shelter in place drills.
An unannounced monitoring inspection was conducted today from approximately 3:45pm-5:30pm There were 11 children (37 points) directly supervised by 3 caregivers. A sample of 5 children?s records and 2 caregiver's records and 1 household member's record was reviewed. The children were observed drinking milk, playing, reading and during diapering procedures. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-111-960-E Description: Based on observation of 3 refrigerated bottles, bottles were not correctly labeled. Evidence: Children B and D were missing the date on their bottles. Plan of Correction: I will date them.
Standard #: 22VAC40-111-60-A Description: Based on 5 children's records reviewed, children's records were in complete. Evidence: 1) Child A was missing an annual update, most recent update was 8/18/2016. 2) Child B was missing the work address of both parents. 3) Child C was missing the insurance policy number and an annual update. The file was last updated 10/28/2016. Plan of Correction: Corrected during inspection.
Standard #: 22VAC40-111-330-A Description: Based on observation and interview, the landline phone was not in compliance. Evidence: The landline phone requires electricity to operate. The provider stated that she does not have a phone that does not plug into the wall. Plan of Correction: I will buy one.
