Technical Assistance:
Discussed placement of changing area in proximity to hand washing.

An unannounced inspection was conducted from 820-935. There were 6 children present with the caregivers for a total of 17 points. The Provider has maintained files for enrolled children, household member, and assistant. The children were observed playing and an arrival was also observed. The downstairs area is set up for children and the Inspector did not observe any hazards. Please call with questions 703-479-4679

Description: Based on record review, the Provider has not reviewed the emergency plan at least annually and documented, in writing each review and whether an update to the emergency plan was needed. Evidence: 1. Last date of Emergency Plan review was two years ago.

Plan of Correction: We will review and make changes if necessary. We will document and date the review.

Description: Based on record review of three children's files, the Provider did not obtain documentation of annual review of child's information. Evidence: 1. Three children's files reviewed and all three have been enrolled for over a year. The record has a space for annual review of information and has not been signed to verify information for the children are current.

Plan of Correction: We will have parents review and sign off to verify information is current.

Standard #: 22VAC40-111-180-A
Description: Based on record review and staff interviews, the Provider did not obtain current report of TB Screenings every two years from the dates of the last screening for herself, a household member, and an assistant. Evidence: 1. Inspector reviewed records for the Provider, a household member, and the assistant. All had dates that were over 2 years and 6 months old.

Plan of Correction: We will all go and have TB Screenings repeated.
