Comments:
The licensing inspector conducted an unannounced renewal inspection from 10:30am - 12:45pm. There were no children in care during the inspection. Two caregiver's records, one household member's record and three children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to maintain the family day home in a clean, safe and operable condition. Evidence: 1. The inspector observed the home. In the front room, which is primarily used for child care, the inspector observed several items on the floor. These items included a penny, a brush, paper, crayons, a plastic fork, a potato, a large spoon, a sock and dirt. In the kitchen, the inspector observed a trash can with items on top blocking the back door. In the bathroom, the sink would not drain.

Plan of Correction: The provider swept the floor and cleaned off the table during the inspection. The provider will move the items and the trash can so it won't block the door. The provider will unclog the sink.

Standard #: 22VAC40-111-460-C
Description: Based on record reviews and interview, the family day home did not have all of their pets vaccinated for rabies. Evidence: 1. The inspector asked to review the health records for the family pets. There was no documentation for rabies vaccinations for three dogs and three cats. 2. The inspector asked the provider if these pets had their rabies vaccinations and the provider said they did not.

Plan of Correction: The provider will schedule appointments for the pets to have their rabies vaccinations. The provider will send copies of the vaccinations to the licensing office as soon as the pets are vaccinated. These pets stay outside and will remain outside until they receive their shots.
