Comments:
Today an unannounced renewal inspection was conducted with the Provider. This visit took place in the morning between 9:30am-1 there were 8 children present with the Provider and 2 assistants for a total of 28 points within the supervision guidelines. The children were observed learning the color green, shapes, and reviewing the alphabet, lunch and nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance record, and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-800-B
Description: Based on review of provider records, provider did not conduct an annual review of the emergency plan. Evidence: No documentation was found of an annual review of an emergency plan.

Plan of Correction: Provider stated that she does not have it.

Standard #: 22VAC40-111-100-B
Description: Based on review of four children's records, the center did not have documentation of a physical in each child's file prior to or within 30 days of first attendance. Evidence: Child #3's record with a start date of 10/17, did not include a physical exam.

Plan of Correction: Provider will get a copy from parent.

Standard #: 22VAC40-111-110-A
Description: Based on review of eight children's records, the center did not obtain documentation of immunizations for a child prior to the child's first attendance. Evidence: Child #3's record with a start date of 10/17 did not include immunization.

Plan of Correction: Provider will get a copy from parent.

Standard #: 22VAC40-111-60-B
Description: Based on review of five children?s records, the provider did not maintain complete children?s records. Evidence: Child #1 & #3's record did not include documentation of two designated emergency contact person's complete address.

Plan of Correction: Provider will ask Parent for information.

Standard #: 22VAC40-111-180-A
Description: Based on documentation review, the provider did not obtain updated tuberculosis (TB) tests or screenings for each household member every two years. Evidence: Household member #1 & #2's record did not include an updated TB screening since 11/18/15.

Plan of Correction: Provider stated that she will have them done immediately.

Standard #: 22VAC40-111-210-A
Description: Based on review of the Caregivers records, caregivers did not have a minimum of 16 hours of annul training. Evidence: Caregiver #1's ecord only had 12 hours of annual training, and was missing 4 additional training hours for 2017. Caregiver #2 & #3's record did not have documentation of 16 hours of annual training.

Plan of Correction: Provider stated that they attended the same 12 hour training with her however she is unable to locate the certificates.
