Comments:
A focused monitoring inspection was conducted today. The provider was present with two preschoolers and an assistant. Four children's files and three adult files were reviewed. There have been no injuries/accidents nor are their any medications on site. Arrival time: 1:30 pm Departure time: 2:35 pm

Violations:
Standard #: 22VAC40-111-830-B
Description: Based on an interview with the provider, it was determined that shelter in place drills are not being practiced a minimum of twice a year. Evidence: There was no documentation of any shelter in place drills being conducted.

Plan of Correction: I will do a shelter in place drill in December.
