54.1-3408 Provider must be MAT certified to administer prescription medication.

Comments:
An unannounced monitoring inspection was conducted beginning at 9:45 AM and ending at 12:50 PM with the provider and one assistant. There were ten children, ages 19 months, 22 months, 2, 2, 3, 3, 3 4, 5, and 5 years old, in care for a total of 21 points. The children were observed playing with blocks and trains, listening to music, and singing. The children were also observed eating lunch which consisted of pizza, green beans, and milk. Five children's records were reviewed. One staff record and three household members' records were reviewed and in compliance. The most recent emergency escape drill was 11/8/17. The last shelter-in-place drill was 6/8/17. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The provider does not administer medication; there were no medications to review. The provider holds current certification in first aid and CPR. Standard #: 22VAC40-111-920
Description: Based on a review of documentation and provider interview, the family day home did not provide a menu for the current one-week period. Evidence: There was no menu available for the week of December 4 through 8, 2017.

Plan of Correction: The provider stated that she will create a two week menu and post it in the family day home.

Standard #: 22VAC40-111-60-A
Description: Based on a review of documentation and provider interview, the provider did not maintain an up-to-date record for each enrolled child. Evidence: Child A and Child B were in care at the family day home and did not have a record.

Plan of Correction: The provider will ensure that all children attending the family day home will have a record.

Standard #: 22VAC40-111-60-B
Description: Based on a review of documentation and provider interview, not all of the children's records were complete. Evidence: Child C's record was missing address and telephone number of each custodial parent's place of work, the name of the medical insurance company and policy number, and addresses and telephone numbers for information for two emergency contacts.

Plan of Correction: The provider will request the missing information from the parent/guardian.
