Technical Assistance:
Discussed background Code changes that became effective 7/1/17 and the upcoming fingerprint requirements for all staff and household member. Please be sure to submit your renewal application prior to the expiration of your license.

An unannounced Renewal inspection was conducted this date. A total of 3 children present (9pts) with provider present. Children were observed free playing on the middle level of the home and singing nursery rhymes. The home was decorated for the holidays. Walk thru of home conducted. The lower level used for programming was equipped with ample toys, equipment and educational materials that were determined age appropriate for the children in care. One crib currently in use. No medications at this time and no allergies at this time. Discussion on the importance of obtaining health information on children enrolled and the need to maintain complete records. A total of 3 childrens records, provider and two household records were reviewed. This inspection began at 9:25am and ended at 11:20am. Thank you for your cooperation today. Please email me:

Standard #: 22VAC40-111-100-A
Description: Based on child record review, a physical exam was not obtained for one child. Evidence: Child #2 began care on 8/16/17. No documentation of a physical exam was found for this child.

Plan of Correction: I will obtain this documentation from the parent this week.

Standard #: 22VAC40-111-60-A
Description: Based on child record review, one record was not maintained and one record was incomplete. Evidence: No record was found for Child #1. Provider was unable to locate the record. Child #2 began care on 8/16/17, no documentation of the child's physician and phone number was found. Child #2 was missing a second emergency contact.

Plan of Correction: I will locate the record or obtain all information required from the parent again.

Standard #: 22VAC40-111-90-A
Description: Based on child record review, immunizations were not obtained for one child. Evidence: Child #2 began care on 8/16/17. no documentation found that the child has been adequately immunized.

Plan of Correction: I will obtain this information from the parent this week.

Standard #: 22VAC40-111-180-A
Description: Based on household record review, subsequent TB screenings were not completed. Evidence: Household member #1's last TB screening on file is dated 6/12/15. Household member #2's last TB screening is dated 8/7/14.

Plan of Correction: Both household members will obtain a TB screening this month.

Standard #: 22VAC40-111-320-I
Description: Based on observation of the physical plant, provider has not complied with recommendations to eliminate fire hazards. Evidence: The smoke detector located at the bottom of the steps on the level where programming is held was beeping during the licensing inspection and requires a new battery.

Plan of Correction: I will replace with a new battery today.
