Reminder to complete and submit your renewal application prior to the expiration of the license (January 15, 2018).

Comments:
An unannounced renewal inspection was initiated today at 2:30pm. The physical plant, 3 caregiver/household member files, 3 children's files, caregiver training, children's activities, emergency procedures, drills and supplies were inspected today. There were 3 children, totaling 7 points, under the supervision of the Provider. Today, the children played outside, had reading time, enjoyed free play, listened to music and took naps. The children bring their lunches to the FDH each day that they are in care. The Provider is not certified in MAT (medication administration training) and there are no children with any medication needs. Standard #: 22VAC40-111-60-B
Description: Based on file review 1 out of 3 children's records does not have all required information. Evidence 1. Child #2 does not have the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: The Provider will ask the parents for this.

Standard #: 22VAC40-111-280
Description: Based on observations, there was a potentially poisonous substance which was not stored in areas inaccessible to children. Evidence 1. There was a bottle of cleaning solution on the back of the toilet in the bathroom that the children use.

Plan of Correction: This will be moved.
