Comments:
The following inspection took place between the hours of 10:30am am through 1:15pm. Today, I met with the Provider on site at the Family Day Home (FDH). At the start of this unannounced monitoring inspection there were a total of 4 children (10 points) with 1 caregivers. A total of 5 children's files and 4 staff/household member's files were reviewed. The children were observed engaged toy plan, while having lunch, bathroom breaks, hand washing, and other organized activities. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-750-A
Description: Based on observation, 7 topical skin products did not meet the stated requirements. Evidence: 7 out of 7 diaper creams and ointments were not labeled with children's names.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-750-D
Description: Based on observation, a topical product was being used beyond its expiration date. Evidence: A lotion for Child #3 expired 05/2016.

Plan of Correction: I will let the parent know that a new cream is needed.

Standard #: 22VAC40-111-830-C
Description: Based on a review, documentation of emergency response drills was not maintained. Evidence: Documentation of shelter in place drills conducted between December 2016 to December 2017 was not available at the time of inspection.

Plan of Correction: I will make sure to document shelter in place drills.

Standard #: 22VAC40-111-920
Description: Based on a review, the menu posted did not meet the stated requirements. Evidence: 1. Upon inspector arrival, the menu posted was not dated. 2. The menu posted did not list substitutions. The provider noted that [per the menu] children in care were to be served chili with kidney beans, corn bread, zucchini, tomatoes and milk; however, inspector observed children eating rice, eggs and fruit for lunch.

Plan of Correction: 1. Corrected during inspection. 2. I will list any substitutions.

Standard #: 22VAC40-111-60-A
Description: Based on a review of records, 1 of 5 children's records was not maintained up to date. Evidence: The last documented date of review for the record of Child #4 was 12/19/16. An annual review was needed on or before 12/19/2017.

Plan of Correction: I will have the parent sign the form.

Standard #: 22VAC40-111-320-H
Description: Based on observation, flammable items were accessible to children. Evidence: An open flame was observed on a shelf within 3 feet of reach of children.

Plan of Correction: Corrected during inspection. I usually light it earlier so that it burns out before the children are in care and forgot today. I will make sure to do it from now on.

Standard #: 22VAC40-111-520-A
Description: Based on observation, a crib was not used for an infant. Evidence: Child #3 [date of birth: 2/20/2017] was observed sleeping in a play pen.

Plan of Correction: We have a crib and I will buy a monitor to keep her in it.
