Comments:
An unannounced renewal inspection was conducted today from 10:40am-1:15pm. There were 7 children (24 points) directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, building with Legos, singing and eating lunch. Today's lunch consisted of chicken, spinach, mashed potatoes, apple sauce and milk. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Violations:
Standard #: 22VAC40-111-800-A
Description: Based on record review and staff interview, the family day home did not have a written emergency preparedness and response plan. Evidence: The provider did not have an emergency preparedness and response plan during the inspection.

Plan of Correction: I will complete the form as soon as possible.

Standard #: 22VAC40-111-830-C
Description: Based on documentation review, the provider did not have documentation of emergency escape drills as required. Evidence: An emergency escape drill was not documented for the month of December.

Plan of Correction: We will make sure we practice and document escape drills every week.

Standard #: 22VAC40-111-60-A
Description: Based on review of children's records, the provider did not maintain up-to-date records at the family day home for each enrolled child. Evidence: Child#4's record did not have an annual update of emergency contact information. The child's record was last signed and reviewed on 12/23/2016.

Plan of Correction: I will ask the parents to review it today.

Standard #: 22VAC40-111-60-B
Description: Based on child record review, the provider did not maintain complete information for each child. Evidence: Child#3's records did not contain the child's medical insurance policy number, an address for 1 of 2 emergency contacts, the parents' work address and work phone number.

Plan of Correction: I will ask the parents to complete the form.

Standard #: 22VAC40-111-70-A
Description: Based on review of 5 children's records, the provider does not have documentation that parents were provided in writing documentation required before the child's first day of attendance. Evidence: Child#2's record did not contain a signed copy of the family day home's Medication Policy, and the Information for Parents Form.

Plan of Correction: I will give the parents a copy of the forms.
