Nine children were in care with the provider and one assistant. The age range of the children was four months to five years old, total points were 22. The children were resting during the inspection. Inspection time 1:00pm-3:00pm
Standard #: 22VAC40-111-210-A Description: Based on review the 16 hours of annual training was not completed. Evidence: Staff A completed 15.0 of annual training and Assistant A completed 13.5 hours of annual training. Plan of Correction: We will complete the required training hours and make up the hours not completed.
Standard #: 22VAC40-191-40-D-1-c Description: Based on review of three records not all had updated background checks. House hold member B, assistant A and the provider did not have updated search of central registry checks. The dates on the checks were12/2/14. Plan of Correction: The Assistant contacted the Search of central registry and both the provider and assistant A checks were mailed out. House hold member B's check needs to be resubmitted.
