54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted today from approximately 10:50am-12:35pm. There were 6 children (22 points) directly supervised by 2 caregivers. A sample of 3 children?s, 2caregiver's and 1 household's member's records were reviewed. The children were observed playing with legos, eating lunch, and during hand washing procedures. One new child began care today. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. Standard #: 22VAC40-111-830-A Description: Based on record review, evacuation drills were not practiced monthly. Evidence: For the month on January 2018, an evacuation drill was not practiced. Plan of Correction: Drills were resumed in February 2018.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: 1 of 3 children's records reviewed were incomplete. 1) Child A was missing the employment information of the father. Plan of Correction: Parents will update the file today.
Standard #: 22VAC40-191-40-D-1-c Description: Based on record review, repeat criminal history checks were not initiated. Evidence: A repeat Criminal History Record check for caregiver A was not initiated. The previous check was returned 10/22/2014 Plan of Correction: I will complete fingerprinting next week.
