Discussed fingerprinting process. Provided copy of TB screening form.
An unannounced monitoring inspection was conducted today between the hours of 9:30 am through 11:00. There were a total of 6 children (16 points) with 1 caregiver. A total of 2 children's files and 1 staff/household member's files were reviewed. The children were engaged in the following: toy play, snack, bathroom breaks, and hand washing. Standard #: 22VAC40-111-960-E Description: Based on observation, bottles are not labeled and dated with infant's name. Evidence: There were 2 infants in care, a bottle was observed not labeled with the child's name or dated. Plan of Correction: I will label each bottle.
Standard #: 22VAC40-111-100-A Description: Based on record review, the provider has not obtained documentation of the child's physical examination within 30 days of first day of attendance. Evidence: Child # 1, date of first attendance 1/2/18 does not have documentation of a physical examination. Plan of Correction: I will ask for the physical examination.
Standard #: 22VAC40-111-60-B Description: Based on record review, child's record is not complete. Evidence: Child # 2 record does not include the medical insurance policy number. Plan of Correction: I will get the policy number form the parent.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider has not maintained a report of TB screening for each household member every 2 years from the date of the last screening. Evidence: Household #1 TB screening is dated 2/23/15, not within the last two years of the date. Plan of Correction: I will have the doctor fill out the TB form.
Standard #: 22VAC40-111-530-B Description: Based on observation, a crib had a loose fitting quilt laying on top of the bottom cover. Evidence: An infant was observed laying in the crib. There was a quilt laying loosely on top of the fitted crib sheet. Plan of Correction: I will remove the quilt.
Standard #: 22VAC40-111-530-F Description: Based on observation, a pillow was being used in a infant crib. Evidence: There was an infant observed in a crib with it's head laying on a pillow. Plan of Correction: Provider removed the pillow.
