Today an unannounced Monitoring inspection took place between 1:00pm and 2:45pm. There were 5 children present with the provider and 1 assistant for a total of 14 points within the supervision guidelines. The children were observed napping. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical exam prior to or within a child's first 30 days of attendance. Evidence: Child #1 did not have documentation of a physical exam on file. The provider has confirmed that the child has been in attendance for more than 30 days.

Plan of Correction: This is leaving in one week.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 file did not contain documentation of the child's first date of attendance. Child #3 file did not contain documentation of the child's first date of attendance, an address for their first emergency contact and a phone number and address for their second emergency contact.

Plan of Correction: I'll give this to the parents tonight.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain updated immunization records every six months for children under the age of two years. Evidence: Child #4 (DOB: 1/27/2017) most recent immunizations on file are dated 3/27/2017. Child #5 (DOB: 3/7/2016) most recent immunizations on file are dated 9/23/2016.

Plan of Correction: I'll talk to the parents tonight.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the caregivers did not obtain a current Tuberculosis (TB) screening every two years. Evidence: The providers TB screening expired on 7/9/2017. The assistant caregiver's TB screening expired on 8/12/2017.

Plan of Correction: My husband can call to see if we can get it done.
