An announced Initial inspection was conducted today. There are 3 children present (5 points) with one adult. 1 adult, 1 dog and 3 children's records were reviewed. See violation notice for missing information. Children were observed playing, washing hands, eating morning snack and cleaning up to go outside. There is one crib assembled for use with a manufacture date of 3/2012. A Subsidy Health and Safety Inspection (SHSI) was conducted on 02/28/2018 to review supplemental health and safety requirements. Description: Based upon a review of first aid supplies, the first aid kit does not contain all required components. Evidence: The first aid kit contains 1 of 2 required triangular bandages.

Plan of Correction: I will purchase another triangular bandage.

Description: Based upon a review of records, one child's record does not contain all required information. Evidence: 1 of 3 records reviewed was missing documentation of the health insurance information (Child 1).

Plan of Correction: I will ask the parent for the health insurance information.
