An unannounced monitoring inspection was conducted on March1, 2018. There was one child present during the inspection with the provider totaling two points. There are two children enrolled in the program. The child was observed having breakfast which consisted of eggs, pancakes, bacon and milk and playing freely during the inspection. No medications are being administered in the family day home at this time. Two children's record and the provider's record were reviewed during the inspection. The outdoor play area is not being used at this time.

Standard #: 22VAC40-111-770
Description: Based on observation and inspection of the facility, the provider did not ensure that there was a working battery-operated flashlight available. Evidence: During the inspection conducted on March 1, 2018, there was not a working battery-operated flashlight.

Plan of Correction: Will purchased today. The provider was on her way to purchase when the inspector arrived.

Standard #: 22VAC40-111-800-B
Description: Based on review, the provider did not review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan. Evidence: During the inspection conducted on March 1, 2018, the last documented review of the emergency plan was dated 8/23/16.

Plan of Correction: The plan will be updated today.

Standard #: 22VAC40-111-60-B
Description: Based on record review, in one of two of children's records reviewed, the provider did not ensure that each child's record contained all required information. Evidence: During the inspection conducted on March 1, 2018, the record for Child #1 did not contain the following information: 1. Written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds; 2. Policy number of the child's medical insurance 3. Phone Number for one parent
