Comments:
An unannounced renewal visit was conducted today between the hours of 9:27am through 11:30am with the provider on site. There were 6 children (15 points) with the provider and one assistant. A sample of 5 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, singing, playing with puzzles and other table toys, bathroom breaks, hand washing, and other organized activities. The interactions between the children and the caregiver were seen to be positive. The home is clean and organized. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-780-B
Description: Based on review, the provider did not review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct and have the parent sign. 1. Child A's file did not have documentation of a yearly review of emergency information with parent. It was last signed on 9/14/2016. 2. Child B's file did not have documentation of a yearly review of emergency information with parent. It was last signed 09/12/2016. 3. Child D's file did not have documentation of a yearly review of emergency information with parent. It was last signed 11/10/2016.

Plan of Correction: I will have the parents review the information, update if needed and sign it.

Standard #: 22VAC40-111-830-C
Description: Based on observation and interview, the provider did not maintain documentation of shelter-in-place drills. Evidence: 1. The provider had documentation of emergency evacuation drills, but did not have documentation of shelter in place drills. The provider stated the shelter in place drills were conducted.

Plan of Correction: I will make sure I document the drills.

Standard #: 22VAC40-111-100-A
Description: Based on review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child E's file was missing results of a health exam and started with the provider on 10/17/2017.

Plan of Correction: I will ask the parent for a copy of it.

Description: Based on review, five of five children's records were not complete with required information. Evidence: 1. Child A's file did not have documentation of a yearly review of emergency information with parent. It was last signed on 9/14/2016. 2. Child B's file was missing proof of identity and name and address of previous child care and did not have documentation of a yearly review of emergency information with parent. It was last signed 09/12/2016. 3. Child C's file was missing one parent's address and phone number and the medical insurance name. 4. Child D's file was missing proof of identity and name and address of previous child care and did not have documentation of a yearly review of emergency information with parent. It was last signed 11/10/2016. 5. Child E's file was missing proof of identity and name and address of previous child care and results of health exam.

Plan of Correction: I will ask the parent's for the information.

Standard #: 22VAC40-111-180-A
Description: Based on review, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department. Evidence: 1. The provider had documentation of a TB test on 2/09/16, but did not have a copy of the last TB screening performed. The provider stated it was done in February 2018.

Plan of Correction: I will get a copy of it.
