Technical Assistance:

An unannounced renewal inspection was conducted on Thursday, March 22, 2018 from approximately 10:45AM to 1:15PM to determine the provider's compliance with licensing standards. In attendance were 10 non-resident children. The children were being supervised by two caregivers during the inspection. There was a total of 26 points based on the children?s ages. The children were observed playing with toys, being held and fed, eating lunch and transitioning into naptime. Second helpings of lunch were provided. The caregivers were observed engaging the children in conversation and completing activities with the children, holding and rocking infants, and providing assistance when needed. A walk through of the home was conducted with the provider. The child care areas were filled with supplies and toys for the children. During the inspection, the licensing inspector reviewed the following: emergency supplies including first aid kit, battery operated weather radio, emergency plan, emergency drill logs, required postings, menu, caregivers? training hours and daily attendance. The provider stated medications are not being administered at this time. Transportation is not provided. During the inspection five children's and six caregivers? records were reviewed. The risk profile tool will be used for the licensing recommendation by the licensing department. Standard #: 22VAC40-111-100-A
Description: Based on review of five children's records, the provider did not obtain a physical exam for one of the children in care. Evidence: During the inspection, the licensing inspector observed the record for Child #2 (start date 11/20/17) did not have a documented physical examination by or under the direction of a physician. Documentation of a physical exam is required for each child prior to or within the first 30 days of attending the Family Day Home.

Plan of Correction: I will contact the family and request a copy of the physical exam.

Standard #: 22VAC40-111-60-B
Description: Based on review of five children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #4 (start date 09/12/16) did not have a documented annual review of the information in the child's records. The form was dated 09/11/16. Parents or guardians are required to review and make any necessary changes at least on an annual basis.

Plan of Correction: I will have the parent review the record today at pickup and make any changes if it is necessary.

Standard #: 63.2-1720.1-A
Description: Based on review of six caregivers? records, the provider allowed one caregiver to supervise children prior to receiving notice of eligibility based on results of a national fingerprint-based criminal history record check. Evidence: During the inspection, the licensing inspector observed the record for Caregiver #2 (hire date 03/19/18) did not contain the required eligibility letter stating the caregiver was eligible to work in a family day home after completing a national fingerprint-based criminal history record check. Caregiver #2 was observed working with the children during the inspection. The provider stated Caregiver #2 completed the fingerprint appointment on 03/07/18 but the results have not been returned as of 03/22/18. As of 01/22/2018, a statement of eligibility based on a national fingerprint-based criminal history record check is required to be obtained prior to staff working in the facility.

Plan of Correction: Caregiver #2 went for the fingerprint background check on 03/07/18. I waited for over a week for the results before allowing the caregiver to work. Based on my enrollment, I determined it was necessary to allow Caregiver #2 to work. Caregiver #2 is being kept under constant sight and sound supervision until the results are returned.
